I hereby grant permission to ESB Academy to use the above-named participants image, likeness, or voice recording on the ESB Academy website or in any other official ESB Academy publication without further notice or compensation. I hereby consent that any such image, likeness, or voice contained in photographs, recordings, or videos are the property of ESB Academy, giving them permission to print, reprint, publish, copy, vend, perform, or represent publicly, free and clear of any claim whatsoever on my part or the part of the participant. 

Participation in all sports can be inherently dangerous and it is impossible to ensure the safety of all participating individuals. Basketball demands cardiovascular fitness, coordination and agility. Although many risks can be avoided, I understand that not all risks can be and therefore release ESB Academy from responsibility for any injury that my child might incur during the course of normal play. I agree to also indemnify and hold harmless the volunteers, trainers and owners of the organization. I further understand that in the event of a medical emergency, ESB Academy will call EMS to render assistance and that I will be financially responsible for any expenses involved.
